*  Complete form and be sure to mark the correct Option 
*  To request and Official Enrollment Verification;
        1.  Bring this completed form and
        2.  Your picture identification and
        3.  $5.00 to Admission and Records.

Option 2:
*  Print your class schedule by logging into Webadvisor selecting "My Class
    Schedule"
*  Click here for Enrollment Verification Status chart
*  For unofficial Enrollment Verification, attach chart to your schedule.
