  1. Complete all blank fields. This form cannot be submitted unless all fields are filled. When complete a "submit" button will appear. If information is not available indicate with N/A. (Incomplete information may result in denial of appeal.)

  2. Explain your situation fully as to why the vehicle was not in violation. The Parking Administrator's decision will be based on your written explanation.

  3. Submit by email or print form and mail to the address at the bottom of this page. Include copies of all applicable documentation relating to your appeal (documentation will not be returned). Example: COD permit receipt; DMV Disabled placard ID receipt.

NOTE: Lack of knowledge is not an adequate reason for dismissal. It is the student's or staff member's responsibility to properly display the current parking permit and park only in authorized parking spaces.
