Financial Aid Forms

Beginning with the 2014-2015 school year many of the forms required by the Financial Aid Office from students can be completed online (e-forms).  Students will simply need to check their required documentation on WebAdvisor and then go to the Portal homepage and under the QuickLaunch Navigation menu on the left side, click on the link labeled, Financial Aid Forms.  Students can select the needed form, complete and submit all online. Please allow at least 2 business days for the forms to be cleared in WebAdvisor.  

Additionally:

  • Parent signatures are still required for dependent students so the form will be emailed to the student after which it can be printed, signed by the parent and returned to the Financial Aid Office. 

  • Students can upload IRS tax return transcripts, W-2 forms, high school transcripts or other forms needed by the Financial Aid Office by clicking on the Supporting Documents Upload link and attaching any scanned document files.

  • Students still have the option to bring in, mail or fax requested documents to the Financial Aid Office as before.  Forms can be picked up from the Financial Aid Office during regular business hours. 

  • Descriptions of documents are provided below for your reference. 

Please note that you may be requested to provide additional information during the review process.  Once you have submitted all of the required forms, the Financial Aid Office can complete the review of your file and award your financial aid.  Please allow sufficient time to complete this process.  It is recommended that students complete their file by July 1st each year to ensure they receive their financial aid funds with the first disbursement in September and qualify for a bookstore authorization.

FORM NAME    DESCRIPTION
Verification Worksheet​ Submit the appropriate Verification Worksheet. Forms may submitted online through your MyCOD portal.​ The worksheet is required along with the student's income information (and their parents if they are dependent') before their file will be complete.

Student Federal Tax Return Transcript

Submit a copy of your (and/or your spouse's) 2012 federal tax return transcript from the IRS.  You can order online by going to www.irs.gov. INDEPENDENT students: If you did not file taxes, please order a Verification of Nonfiling letter from the IRS.

Parent Federal Tax Return Transcript

Submit a copy of your parent(s)' 2012 federal tax return transcript from the IRS. You can order online by going to www.irs.gov.

SNAP Form​
Submit the SNAP Verification form. Form may be submitted online through your MyCOD portal or picked up at the Financial Aid Office.  
Student Income and Expense Sheet​ Submit the Student Income and Expense Sheet. You may pick up a copy at the Financial Aid Office.
Identity​ Submit the Identity and Statement of Educational Purpose form.​ Form may only be picked up at the Financial Aid Office.
Child Support Paid​ Submit the Child Support Paid form.​ Form may be submitted online through your MyCOD portal or you may pick up a copy at the Financial Aid Office.
FERPA​ In accordance with federal law and the Family Educational Rights and Privacy Act (FERPA), the Financial Aid Office may only release student financial aid records directly to the student, unless prior written authorization is given by the student. By filling out this form you give permission for others to view and have access to your student records. Form must submitted in person with government issued ID, if mailed, must be notarized.

Parent's W-2's

Submit copies of your mother's/father's 2012 W-2's.

​DREAM App Worksheet ​Select AB540 students who have completed the CA Dream Application and who are receiving Cal Grant need to submit the appropriate worksheet along with their income information (and their parents if they are 'dependent') before their file will be complete. Form may be submitted online through your MyCOD portal or you may pick up a form at the Financial Aid Office.

Student (and/or Spouse) W-2's

Submit copies of your (or your spouse's) 2012 W-2's.

Copy of HS Transcript Needed

Need to submit a copy of your high school transcript (must have graduation date), GED scores or the equivalent.

Selective Service

According to Selective Service records you are not registered as required by law. You must submit proof of Selective Service registration or a Status Information Letter.  You may register at www.sss.gov Additional information may be requested by the Financial Aid Office.

Student Loan in Default

Our records indicate that you are currently in default on a federal student loan.  You must resolve the default with your lender and provide a default clearance letter verifying that you are no longer in default. You may also call the Department of Education at 1-800-621-3115.

Proof of Current Active Duty

Submit documentation that you are currently serving on active duty in the U.S. Armed Forces for purposes other than training.

Social Security Citizenship

Need documentation/proof of your U.S. citizenship which includes a copy of your U.S. birth certificate, U.S. passport or naturalization certificate.  

Proof of Dependent(s)

Provide documentation that you have dependent(s) who live with you and who receive more than half their support from you, as of now and through the end of the school year for which you are applying for financial aid.

Need Copy of Death Certificate

Need a copy of your mother's and/or father's death certificate.

DHS Verification

Need documentation/proof of your US citizenship or eligible non-citizen status.

Copy of Divorce Documentation

Submit divorce documentation for you (or your parents?).

Proof Emancipated Minor Status

You need to submit a copy of a court's decision from your state of legal residence that you are or were an emancipated minor.

Need Further Information

You are requested to submit further information on documents submitted please contact the Financial Aid Office for more information at (760) 773-2532.

Proof Orphan/Foster Care/Ward

Need documentation indicating that both your parents are deceased or you were in foster care or you were a ward of the court at any time since you turned age 13.

Parent Conflicting Tax Info

Your parents' tax information conflicts with your FAFSA data - resolution needed.

Parent's Drivers License

Need a copy of your mother's and/or father's driver's license.

FAFSA/ISIR Reject

The information you submitted on your FAFSA was rejected by the federal processor. Please correct and resubmit to the Dept. of Education.  You can also call the Financial Aid Office at (760) 773-2532 for additional information.

Student Driver's License

Need a copy of your driver's license.

Student Educational Plan

You need to complete a Student Educational Plan (SEP) developed in consultation with a COD academic counselor which lists all course requirements and the timeline for achieving your educational objectives and submit a copy to the Financial Aid Office.

FAFSA/ISIR Signature Needed

Need student/parent signature(s) on the enclosed ISIR Signature Page. Contact Financial Aid Office or go to www.fafsa.gov and follow instructions for you and/or your parents signing the FAFSA.

Social Security

Need documentation/proof of your correct Social Security Number. As such, bring your Social Security card, driver's license and birth certificate or U.S. passport or naturalization certificate to the Financial Aid Office for verification.

Academic Transcript(s) Needed

Need academic transcript(s) from colleges previously attended. Contact the Financial Aid Office.

Unaccompanied Youth/Homeless

You will need to submit a copy of an official determination that you were designated as an unaccompanied youth who was homeless or at risk of being homeless at any time on or after July 1, 2012.

​Veterans Affairs Status Submit documentation/proof of your veteran status (DD-214, etc.)
