Please review the Upward Bound Requirements (Part 1) to determine if the student/family is eligible for the program. Print and complete the application (Part 2) and submit to Upward Bound along with a copy of the student's Social Security Card (Signed), Birth Certificate or U.S. permanent Residency Card, Current School Grade Transcripts (Report card for 8th graders) and a signed copy of parents' most recent Federal Taxes (Form 1040). Print out and ask a Teacher or Counselor to complete the Recommendation Form (Part 3).
