AB 540 Form Information

Instructions for completing and submitting the AB540 form:

Requests without attendance dates, total credits, or missing photo identification will be denied. 

1. Download and complete AB540 form using the button below. 

• You will be required to provide information on the schools you attended, including the dates you attended and the number of credits or hours obtained.

2. Submit the completed form along with your photo identification in person at an enrollment services office or via email at enrolls@collegeofthedesert.edu.
