Legal Name Change
                        During peak processing please allow up to 10 business days for the form to be reviewed
                           after all requested documents have been received.
                        
                        It is recommended that students submit their requests in a timely manner to ensure
                           there are no delays with their enrollment records. For additional questions, please

                        To be approved the Legal Name Change request, all documents listed below are required: 
                        
                        
                           
                           - Copy of photo identification reflecting your previous name
 
                           
                           - Copy of valid government issued photo identification reflecting your new name (driver's
                              license, state ID card, passport, etc.)
 
                           
                           - A legal, court filed document showing support for name change, such as marriage license,
                              divorce decree or court order.
 
                           
                           - Copy of signed Social Security Card issued to your new name (applies to students with
                              SSN on file)
 
                           
                        
                        All documents listed above must be uploaded prior to submitting the legal name change
                           form. Requests that are submitted without all documentation uploaded will be denied.
